To select a medical alert system, start by assessing the specific needs and skills of your loved one, taking into account both how they are now and how they could change in the future. For example, if you have dementia, would you understand how to manage a system? Or would it be more appropriate to have something automatic, such as a device to detect falls? Do you have a disorder, such as aphasia, that would make it difficult for you to contact a call center?

And keep in mind that for any of these devices you can ask if a free trial period is offered so that you and your loved one can see what works best.

1. What you need the system to do

  • Ask for help. Devices that are worn and have buttons that are pressed for help can connect to a real person or directly to the emergency services (fire department, police).
  • Detect or prevent falls. Each year, 1 in 3 adults over 65 falls. This is perhaps the most common reason to obtain a medical alert system.
  • Monitor health. This includes reminders to take medications and monitoring vital signs.
  • Detect and track the location through GPS navigator. It is useful if your loved one still drives and is quite independent.
  • Monitor the activities. Motion detectors and sensors track movements in the home.
  • Provide daily contact services. They get in touch through a real person or electronically.
  • Track the exercise. They incorporate step counters and tools that provide information, challenges to improve health and virtual family connections.
  • Monitor home security. For fires, smoke and carbon monoxide.

2. The type of equipment that would work best 

  • Can you wear it? Is the device comfortable (watch out for sharp edges or webbing materials that could irritate delicate skin)? Is it attractive or discreet enough that your loved one will be willing to use it?
  • How waterproof is it? Can it be used in the shower? Can you completely immerse yourself in water, in the sink or bathtub? Many falls happen in the bathroom or kitchen, which is why this is essential.
  • What are its reach, mobility, and connectivity? Ask how far the device will work from the base. Will it work in the yard or garage? Does it include a GPS navigator, so it will work wherever you go in the community? Does it connect to a smartphone or via Bluetooth?
  • Is it transportable? If your loved one moves, can the system move with him?
  • Does it include a safe? Some companies offer to install a safe that emergency medical professionals can use if they need to enter the home when the resident is disabled.
  • Can family members connect to the device? Can you verify what is happening through a smartphone, tablet or computer?

3. Details on response and monitoring 

  • Response Center The average response time should be in seconds, not minutes. Does the company operate its own response center or have an external contract? Is the response center certified? How are operators trained? And can they communicate in the language your loved one prefers? Can your loved one talk to a person through the device he is wearing, or should he be close to the base to be heard?
  • Where the calls are directed. Can you tell where you want the alerts or calls to be directed according to their type (urgent, non-urgent, emergency), including a response center, family or friends, or directly to the emergency services (police and fire)?

4. Cost 

  • Charges Are careful of complicated pricing plans and hidden charges. Find a company that has no additional charges related to equipment, shipping, installation, activation, or service and repairs. Do not be fooled by free service or “donated or used” equipment.
  • Contracts You should not have to sign a long-term contract. You should only have to pay monthly charges, which should vary between $ 25 and $ 45 per month (approximately $ 1 per day). Be careful if you pay for the service in advance, because you never know when you will need to suspend the service temporarily (for example, due to hospitalization) or permanent.
  • Warranty and cancellation policies. Look for a money-back guarantee or at least one trial period, in case you are not satisfied with the service. And you should be able to cancel at all times without penalties (and receive a full refund if you had already paid the monthly charges).

5. Availability in your area 

Many national companies offer medical alert services. But maybe not everyone is available near you, so call and ask about service areas. Local companies may also be an option. In addition to the companies that have been involved in the medical alert business for decades, now more and more technology and home security companies also offer these services.

  • Start an online search. Use keywords such as “medical alert systems”, “personal emergency response systems”, “fall detection devices” and “urgent response devices” ( emergency response devices), along with the name of your city or state, to find companies that offer service in your area.
  • Contact your Area Agency on Aging. Ask if they have a list of companies that offer medical alert services available locally. (I contacted mine, and they immediately emailed me a list of 16 national and local companies, including one offered through the Area Agency on Aging. Find your local agency at eldercare.gov, available in English).
  • Check with your senior center. If you or your loved one lives in a center or community for older adults, you may offer an internal or external medical alert system as part of your general services. Beware of centers that only have pull ropes in a few places in the room or apartment. Too often, people do not fall or get sick in a place from where they can reach this rope.
